Cortisol is your body's primary stress hormone - and when its rhythm is disrupted, it
affects nearly everything: your energy, sleep, metabolism, immune function, hormones,
and mood. Naturopathic physicians Rebecca Sand ND and Elizabeth Emrick ND at
Open Wellness PDX specialize in identifying and treating cortisol dysfunction, including
HPA axis dysregulation (sometimes called adrenal fatigue), elevated cortisol, and
blunted cortisol patterns. If you feel wired but tired, exhausted but unable to sleep, or
like your body's stress response is permanently stuck in the on position, this work may
be for you. Call the Portland office, request a telehealth visit, or book online.
Cortisol is produced by the adrenal glands in a daily rhythm - peaking in the morning to support energy and focus, and dropping in the evening to allow for sleep and recovery. When this rhythm is disrupted by chronic stress, illness, poor sleep, or other systemic imbalances, the result is HPA axis dysregulation - a pattern that shows up differently in different people.
Some experience chronically elevated cortisol: anxiety, poor sleep, belly fat accumulation, blood sugar instability, and immune suppression. Others develop blunted or flattened cortisol output: profound fatigue, difficulty waking, low resilience to stress, and slow recovery. Many people cycle between both.
Common symptoms of cortisol dysfunction include morning fatigue with a second wind late at night, difficulty falling or staying asleep, anxiety and irritability or low stress tolerance, brain fog and poor concentration, cravings for salt, sugar, or caffeine, abdominal weight gain, frequent illness or slow recovery, and worsening PMS, irregular cycles, or thyroid symptoms.
Cortisol dysregulation is typically driven by sustained demand on the HPA axis - more than the body can recover from without support. Contributing factors include chronic psychological stress, inadequate sleep, gut infections or dysbiosis, blood sugar instability, overtraining, nutritional deficiencies, and underlying thyroid or sex hormone imbalances. Advanced lab testing using the DUTCH Complete test allows Dr. Sand and Dr. Emrick to map out the full cortisol curve, its metabolites, and downstream hormone impacts with precision.
Treatment is highly individualized based on each patient's cortisol pattern and contributing factors. It may include adaptogenic botanicals such as ashwagandha, rhodiola, and eleuthero; targeted nutrients including vitamin C, B5, magnesium, and phosphatidylserine; amino acid support; nutrient optimization; therapeutic sleep protocols; blood sugar stabilization; gut health treatment; and nervous system regulation practices. Dr. Sand and Dr. Emrick also assess and address downstream effects on sex hormones, thyroid function, immune health, and metabolism that frequently accompany cortisol dysfunction.
