Perineural Injections Q&A

What is perineural injection therapy?

Perineural injection therapy (PIT) is a drug-free injection that treats chronic neuropathic pain and uncomfortable nerve inflammation. The injection itself is made up of a mixture of dextrose, a type of simple sugar, and sterile water, so it’s safe and harmless for your body.

PIT is primarily used to target small nerves that don’t show up on MRIs or X-rays or superficial nerves that sit just beneath the skin’s surface. The injection reduces nerve inflammation, which helps relieve chronic pain at the site.

For what is perineural injection therapy used?

At Open Wellness PDX, Dr. Chang recommends perineural injections to help reduce inflammation and alleviate the symptoms of the following conditions.

  • Back pain
  • Whiplash
  • Golfer’s elbow
  • Chronic regional pain syndrome (CRPS)
  • Neck pain
  • Groin pain
  • Achilles tendinosis
  • Tennis elbow
  • Sports injury nerve pain
  • Post-surgical pain
  • Nerve entrapment syndrome
  • Arthritis
  • Bursitis
  • Sciatica
  • Muscle and ligament strain

What sets perineural injections apart from other alternative treatments and therapies is that a single treatment can help reduce inflammation instantly, so the effects may be felt right away.

How do perineural injections work?

Once you and Dr. Chang decide that PIT is right for you, you can come into the office for your first series of injections. He performs the perineural injections with very small needles, so they shouldn’t feel painful. However, tenderness, soreness, and bruising at the injection site is perfectly normal.

Depending on the severity of your condition, Dr. Chang may administer several tiny injections to ensure he effectively targets every inflamed nerve. Many patients feel instantaneous pain relief once their nerve inflammation begins to subside.

Since PIT uses a solution of diluted dextrose, it’s completely safe to return for additional treatments. Dr. Chang recommends at least two to eight treatments, spaced out at least one to two weeks apart, for sustainable pain relief.

After a series of perineural injections, most patients report approximately 80-100% total pain resolution. To maintain your results, Dr. Chang may recommend returning to the office for occasional maintenance treatments.
