If you've recently been told you have IBS (irritable bowel syndrome), you're probably left wondering: what actually causes this, and how do I fix it? IBS is best understood as a functional gut disorder — meaning the structure of your digestive tract is typically normal, but the way it functions is impaired. Here's what's actually happening, and what you can do about it.
IBS is a collection of symptoms involving how the gut moves, senses, and responds to food, stress, and inflammation. Most people with IBS experience some combination of:
IBS isn't a "we couldn't find anything else" diagnosis. It's diagnosed using an established set of symptom criteria (the Rome IV criteria) — a recognized, positive diagnosis based on your symptom pattern, not just a process of elimination. Your doctor may run basic tests to rule out conditions like celiac disease or inflammatory bowel disease first, but IBS itself stands on its own.
There's no single root cause — IBS develops from overlapping imbalances in the gut and nervous system. A few of the biggest contributors:
Gut-brain axis dysregulation. Your gut and brain are in constant communication through nerves, hormones, and immune signals. When this signaling becomes sensitized — often after stress, infection, or inflammation — the gut can become more reactive to normal stimuli like food, gas, or movement.

Stress, infections, food intolerances, and hormonal shifts can all act as triggers or amplifiers within this system — which is why IBS symptoms often feel so unpredictable, fluctuating with meals, sleep, stress, and even time of day.
The encouraging news: IBS responds well to treatment when the underlying drivers are addressed. There's no single cure, but most people see meaningful improvement with a layered, individualized approach.
Start with diet and lifestyle. Regular meals, less caffeine and alcohol, good hydration, and regular movement make a real difference for many people. Some patients benefit from a temporary low-FODMAP approach — reducing certain fermentable carbohydrates that trigger bloating — while others are more sensitive to dairy or gluten specifically. The goal isn't long-term restriction; it's identifying your pattern and working with a dietitian experienced in IBS when possible.
Support the gut microbiome. This might mean reintroducing beneficial bacteria through diet or probiotics, or targeted treatment if SIBO or an infection is identified. Your provider can help determine whether testing makes sense for you.

Work on motility. Since IBS-C, IBS-D, and mixed presentations respond differently, this is often where individualized care matters most.
IBS is not "just stress," and it's not "all in your head" — it's a real, physiologic pattern involving your microbiome, nervous system, immune system, and gut motility working together (or not quite in sync).
