As a courtesy, our office verifies insurance benefits prior to appointments and does our best to provide accurate information. However, insurance verification is not a guarantee of coverage, payment, or network status. Some services offered by our clinic may not be covered by insurance, and anticipated out-of-pocket costs will be discussed with you whenever possible. Patients are ultimately responsible for any balances not covered by their insurance plan.
We strongly encourage patients to contact their insurance company directly prior to scheduling to confirm network participation, covered services, deductible information, and benefit details. When speaking with your insurance company, please reference your provider’s individual NPI number listed below, as network participation can vary not only by provider, but also by the specific plan selected under an insurance carrier. While a provider may be considered in network with a particular insurance company, they may not be in network with every plan offered under that carrier. Coverage and reimbursement are ultimately determined by your individual policy and plan benefits.
